In loving memory of John West known as Jack Jack sadly passed away at Wexham Park Hospital on February 21st 2015, aged 95 All who knew Jack are welcome at his funeral on Friday March 13th at 1.30 p. m at Slough Crematorium. Family flowers only but donations, if desired, to AGE UK or Alzheimers Society. To be collected at the funeral or c/o Arnold Funeral Service, 273 Farnham Road, Slough, SL 2 1 HA. PROOF
